Welcome to Providence Health Care, a licensed provider of group homes and residential services for adults diagnosed with intellectual and developmental disabilities who cannot live independently.
We provide a safe environment for up to six residents in a group living situation. staff are provided 24 hours a day to assist and train each resident. Come take a look and be a part of our community

Our Group Homes, or supervised group living, provide a safe environment for up to four residents in a group living situation. Staff(s) are provided 24 hours a day to assist and train each participant in developing daily living skills and to ensure the health, safety, and welfare of the participants.
Each Group Home participant develops an annualized Individualized Support Plan with his/her support team. Support teams are typically comprised of the participant, family members, and staff from a variety of disciplines who provide services. The Individualized Support Plan includes the participant’s goals, likes, interests, abilities, and supports needed. Group Home programming is enacted by home & community program managers and direct support professionals.
